Create the first administrator (first-install only)

/v1/setup/admin

Creates the first administrator — a super_admin — during the first-install wizard and signs them in: returns an access token and sets the operator refresh cookie. Returns 409 Conflict once any user exists. Email verification is bypassed because this endpoint runs before SMTP can be configured.
